By completing a video, photo, and audio release agreement, you grant the FSU Office of Digital Learning permission to photograph and/or record your likeness or voice. If you are granting permission for yourself, select the General Release Agreement. To grant permission on behalf of a minor (child under 18 years of age), select the Release Agreement for Minors.

Use this form if you are granting permission on behalf of a minor (under 18 years of age), are the minor's parent or legal guardian, and are over 18 years of age. If you are completing the form for more than one child, please submit a separate form for each.
